Who operates this website?

Oasis Living is operated under The Trustee for Wealth Guardian Trust, ABN 17 898 768 952. Simon Wu is the initial point of contact at info@oasisliving.com.au.

What the initial form asks for

The referral form asks for referrer/contact details, a participant's preferred name where applicable, general service and funding interests, and optional location, housing and communication preferences. A brief non-clinical note is optional. These details help start a conversation; they do not establish eligibility or a service agreement.

Do not supply NDIS numbers, dates of birth, diagnoses, full addresses, medical records, behaviour support plans, financial documents or other sensitive identifiers. No document upload is provided. Speak to Simon about an agreed secure intake process before sharing detailed records.

How referrals are delivered

Referral delivery is still being configured. You can explore the form, but cannot submit personal information or create a referral through it yet. The existing contact enquiry continues to use the email-draft process described below.

Consent and referring someone else

Before continuing, confirm that the participant agrees or you have authority to act for them, allow Oasis Living to contact you about the referral, and acknowledge that services and accommodation are in development. Consent boxes are not pre-selected. Do not share another person's details without their agreement or appropriate authority.

Website drafts and general enquiries

Form entries are held in the page's memory while you complete it. They are not saved in localStorage or sessionStorage and will be lost if you reload or leave the page. The homepage enquiry form prepares an email in your own email application; it does not send automatically or submit to a website database. Nothing is sent until you send the email yourself.

Hosting, external assets and resources

The website uses hosted assets and external fonts. Standard technical access logs may be processed by the hosting services. We have not added advertising analytics or a marketing signup. External resource links open independent NDIS and NDIS Commission websites, which have their own privacy notices.

Questions, corrections or withdrawal

Contact Simon to ask about the use of your information, correct an initial enquiry, withdraw contact permission or discuss access to or removal of a referral record where applicable. Do not email a clinical record or identity document to verify a request; agree an appropriate process first.

Service status and urgent help

Oasis Living is developing its service capabilities and accommodation model. NDIS provider registration is being pursued. Services requiring registration, SDA certification or dwelling enrolment will commence only after the applicable requirements have been satisfied.

The website is not monitored for emergencies. Call 000 if someone is in immediate danger.
